Recent rapid responses

Rapid responses are electronic letters to the editor. They enable our users to debate issues raised in articles published on bmj.com. Although a selection of rapid responses will be included as edited readers' letters in the weekly print issue of the BMJ, their first appearance online means that they are published articles. If you need the url (web address) of an individual response, perhaps for citation purposes, simply click on the response headline and copy the url from the browser window.

I'm heartened by AstraZeneca finally being taken to task for the harmful effects of Seroquel in causing diabetes. I am waiting for the first of what is sure to be many lawsuits for the off label promotion of Seroquel for agitation and psychosis typically related to delerium in the elderly. "WARNING: Increased Mortality in Elderly Patients With Dementia- Related Psychosis: Elderly patients with dementia-related psychosis treated with antipsychotic drugs are at an increased risk of death. Analyses of 17 placebo-controlled trials (modal duration* of 10 weeks), largely in patients taking atypical antipsychotic drugs, revealed a risk of death in drug-treated patients of between 1.6 to 1.7 times the risk of death in placebo-treated patients."[see reference 1]

Certainly the science is in, the journals have published the results. The drug reps still prey on the misinformed.

In the light of this news report, should we now err on the side of caution when prescribing Quetiapine to those who are diabetic or at risk of developing diabetes? Looking at the BNF (58;September 2009), diabetes does not seem to be a specifically listed side-effect;nor is diabetes identified as a contraindication. Listed under 'cautions';hepatic impairment, renal impairment, pregnancy and cerebrovascular disease.I am interested to know the view of others. (I have not read the full text of this news report)
