    When I was a GP trainee in the east end of London I was asked to see a patient.

    “My sister doesn't seem very well doctor, I can't really explain why but I think she needs a visit. She has cancer you know.” It was late, about 10 pm, and I went along not knowing quite what to expect. I felt slightly guilty about being irritated at having to visit someone who was dying so late at night, but knew that she definitely warranted a visit.
