US doctors' managed care plan folds

    California Advantage, the troubled health maintenance organisation created by 7600 doctors, has filed for bankruptcy. The organisation, a subsidiary of the California Medical Association, comes to an end after three years, three chief executives, and $11 m (£7m) in losses.
