Connoisseurs of the debate on the length of surgical training will enjoy an account of one naval surgical trainee's exchange fellowship to Toronto's Mount Sinai Hospital (J Royal Naval Medical Service 1997;82:183-187). Like many of its counterparts, this institution succeeds in equipping surgeons for independent practice in a five year residency. Should defence medical services emulate this? The argument centres around one's view of what constitutes a ‘general’ surgeon.
