Research is needed into organisation and development as well as cost effectiveness

    EDITOR,—I agree with Sasha Shepperd and Steve Iliffe that the cost effectiveness of hospital at home services is still in question.1 I am less sure that we will know the answer in 1997 when the results of randomised trials become available. The heterogeneity of the schemes will greatly reduce the generalisability of the randomised trials, which, to be feasible, have had to be restricted to looking at patients with selected conditions.
