    “I done me Gazza ligament, innit, doc?” He had been route marching with the Territorial Army. I examined the knee, diagnosed simple overuse, and gave standard advice.

    “No x ray then, doc? Don't get no letter for the specialist?”

    “No x ray; no specialist,” I confirmed. He shrugged, hesitated, and asked for a chest x ray instead.
