    As a consultant general surgeon who has recently completed three years as medical director of a large acute hospital group I would like to offer some advice to aspiring surgeons working in the NHS in the mid-1990s. The NHS Management Executive is creating a climate where consultants are under increasing pressure to fulfil performance targets rather than concentrate on good patient care.
