Klein's elegant editorial describes conflict between different policy
aims in the NHS.
Lord Darzi's proposals for the NHS contain the laudable aim of
rewarding higher quality practices for taking on more patients. This policy
contains an obvious conflict.
Many GPs like myself pride themselves in providing a
personal, accessible service and evidence is abundant, that that the three
"A"s of availability, approachability and affability are what patients
value most, especially in a doctor with whom they have a long term
relationship.
If I take on larger numbers of patients I will become less
available.I suspect that I will also become less affable.
