In reducing this letter from its original rapid response we contracted the original second and third paragraphs so that it no longer made sense. An important sentence (the second in the quotation below) was omitted, for which I apologise.

The first four sentences of the second paragraph of the letter should have read:

"During the last century, when not working in Africa, I did locums in all parts of the country from St Austell to the Orkneys. When I worked in the country I regularly did surgeries in the front rooms of cottages and once in the church hall. I suspect that many such informal arrangements still exist. I hope so."
