Editor For any reform to be successfully implemented it is necessary to carry at least the bulk of those affected with you. On this podcast I have not heard more than platitudes about primary care and rural primary care in particular. In Northumberland currently the CT has publicly declared a desire to eliminate 25% of GPs, end PMS contracts and impose identical capitation funding on all practices. Without a shadow of doubt this will strip primary care out of rural areas entirely and even the largest market towns don't have the critical mass of population or GPs to sustain a polyclinic. All the PMS service developments will be summarily junked and it will be back to 1990s style bog standard primary care provision for all - if you can get to it. Urban solutions do not fit in rural areas. We urgently need to have a dedicated rural medical services review. How about it Lord Darzi? Yours sincerely Steven Ford Competing interests: I am a GP |

May I refer to the interview which I have just heard. In regard to the reference "to meeting the challenges of the next 10 years",my views are not only has the Health Service, its Administrators and Political links been completely negligent regarding the following subject, but deliberately buried and ignored a most essential cause of diseases and deaths in major City locations. Directors of Public Health, the CMO have all denoted their complete lack of interest in traffic related toxins which are being ingested in large daily dosages, with the resultant serious diseases and fatalities occuring. I noted the interview bias towards London, however this expected reference ignored the most severest region affected by such Environmental degredation that of Greater Manchester. This large compacted urban area leads the UK in Cancer rates,Heart diseases, and a host of other diseases created by PM2.5 Particulates and Nitrogen Dioxide being constantly ingested. I reside in a location where 47,000 people live in a compact urban area,an already Government designated AQMA location where Government targets for Air Pollution toxins are breached each and every day,and deliberately increased day by day based on the gross neglect of local Councils,Government avoidance, and a total lack of interest by the medical profession. Epidemiological studies in almost every Country in the World conclusively prove and link Cancers,Heart,Lung,Neurological damage, organ degeneration, foetus damage, and retarded lung growth in the under 21`s as my correspondence with leading research teams confirms. Particulates once ingested cannot ever be expelled are transmitted from lung tissue into the blood stream, and result in free radicals capable of organ disease and degeneration, and all of these facts are buried with the many thousands who die each year from Traffic Pollution related diseases in the UK In Greater Manchester life expectancy is between 7-10 years less than the national average,it has the most traffic congested roads in the UK,it has the lowest tree cover in Europe, and the persistant impact is increasing deaths and serious diseases. I challenge as I have done, any Minister,Director of Public Health,or Sir Lliam Donaldson to leave their enclaves and see the reality of 220,000 plus daily vehicles funnelled from 24 lanes into the M67/M60/A57 major highway interchange, where they are stop/started as close as 65 metres from many family homes spewing out horrendous levels of toxins. I have heard the same response within my previous discussion from Tony Blair,I have received from the CMO similar replies, and I have over a period of 13 years with correspondence with senior influencial leading figures confirmed a total lack of interest shown to this deplorable situation, which is increasing and causing considerable known suffering. Air Pollution monitoring is avoided in pollution "hot spots"for obvious political reasons, and its time someone introduced into senior administrative posts in the NHS people prepared to expose with contempt what is occuring. Directors of Public Health have confirmed that this subject is not even on their agenda`s,the Epidemiological reports from around the world are ignored,as are the WHO reports, and the persistant objective in the UK of how can I preserve my own position is the ultimate situation sought. What can be done ?,well the Planning system completely obsessed with Commercial thoughts should be monitored by regional boards of Public Health advisors, who monitor the impact of large Planning decisions on human health. Please do not respond with that already occurs, local Councils observe these basics within their proceedures, and Regional Assemblies also respect these problems, well my vast volumes of correspondence conclusively prove they do not give a thought to what their decisions will bring to human health, and after 13 years of meetings, from Politicians downwards if they are thought to be the pinnacle of knowledge,I would certainly be happy to address in open debate the facts I have referred to. At present I a deeply involved in another large Public Inquiry and within the next few months will again expose the facts not the rhetoric concerning this buried information So with sincere respect, this is one E.Mail of thousands circulated which will be completely ignored for vested interest reasons, and whilst it drops into a waste bin it will coincide with someone`s death from the Toxins which have originated their disease simply "its not our problem".
