I agree that screening everyone at 36 weeks by ultrasound would be expensive and impractical but what about a vaginal exam if the presentation is unclear at 36 weeks? It is fairly easy to confirm or otherwise a fetal head on a vaginal examination. It is quick, can be done in the office and while it probably doesn't prevent all ultrasound examinations for presentation it may avoid some.
