Dr Jelley must be congratulated on this piece which beautifully captures the essence of general practice in an ever changing world. Budgets, performance, access targets and quality must all be managed in today's NHS but ultimately the true value of general practice is continuity. It is true that, on occasion, depending on lay-referral mechanisms, patients may want 'any doctor now' rather than their own doctor later, but the level of high quality close personal care as described by Dr Jelley is one of the best things about this country's healthcare system. Sadly, current market forces in primary care, changing demographics and constant distortion of patient expectation to unrealistic levels by the government will eventually destroy it. As a result patients, and both primary and secondary care practitioners will suffer - a lose lose for all. Competing interests: I am a GP |

I found this a moving article which described the best of general practice. I hope that this type of care will survive despite the way that government targets and managerialism are eating away at the ethos of our profession. If one looks at the progress of this concept of a 'pateint-led NHS' on the DoH website you will see no reference to patients. I believe this is another example of spin, some bright young advisor's idea of how to get the public on board with a catchy phrase but which has not been thought trhough in any detail whilst the marketisation of the NHS continues apace. As a profession we need to stand up for our values and experience and understanding of how to provide a health service to all who need it .
