My main question related to the paper submitted by Jayawant et al, is, what is the purpose of this paper? It provides almost nothing that is not already published in the current body of literature. The paper seems to refer to American reference material only. On the other hand it totally ignores the paper published by Howard, Bell and Uttley in the British Journal of Neurosurgery entitled 'The pathophysiology of infant subdural haemotomas' which comes to some very different conclusions.

The critical importance of acceleration/deceleration forces and impact in the development of subdural haemorrhaging is hardly mentioned. Whilst the concept of formulating algorithic procedures to aid diagnosis of Shaken Baby Syndrome / Shaken Impact Syndrome, as undertaken by Duhaime et al, is conspicuous by its absence.
