Information given on consent form

This pilot study wishes to assess the validity of two screening questions in a general practice population in terms of their ability to screen patients for depression. We are interested in giving a number of questionnaires to patients visiting their GPs whether or not they feel depressed at the time. That is we wish to give these questionnaires to all patients visiting their GP in a particular week irrespective of why they have come to see their GP.
