In this editorial by Lorraine E Ferris, the 13 reference numbers within the text somehow "dropped off" the page proofs and so did not appear in the version published in the printed journal or in the pdf (which is the same as the print version) on bmj.com (BMJ 2007;334:706-7, 7 Apr, doi: 10.1136/bmj.39168.644757.BE).
