When are randomised trials unnecessary? Picking signal from noise

Paul Glasziou and colleagues used treatment for organophosphate poisoning as an example of a rapid effect that is easy to show (BMJ 2007;334:349-51, 17 Feb, doi: 10.1136/bmj.39070.527986.68). Success requires treatment with anticholinergic drugs, not with cholinesterase inhibitors as suggested.
