EDITORIt is not clear why the UK Beam Trial Team recommends manipulation alone for back pain because it costs more per quality adjusted life year (QALY) than exercise and manipulation combined and even slightly more than exercise alone.1 The average effect of exercise and manipulation combined is also larger than the other two in absolute terms.
