An oversight during our complex ELPS (Electronic Long, Paper Short) system resulted in several errors in the numbering of the reference list of the abridged version of this paper by Anna Holdgate and Tamara Pollock (12 June, pp 1401-4). References cited within the text or figures as 4, 12, 21, 23, 25-28, 30, 31 and 33 link in fact to references listed as 2, 9, 15, 17, 19-22, 24, 25, and 27 respectively. The referencing in the full version of this paper (on bmj.com) is correct.
