Surgery is option for unresponsive temporal lobe epilepsy

Patients with temporal lobe epilepsy should be considered for surgical treatment if their seizures can't be controlled by more than two consecutive anticonvulsants. Weibe and Nicolle (p 656) discuss such recent advances in neurology, which also include using interferon in patients presenting with their first demyelinating episodes to prevent progression to clinically definite multiple sclerosis. Other advances include accurately predicting the risk of stroke in patients with atrial fibrillation by assessing other risk factors, such as congestive heart failure, and treating patients with ischaemic stroke with thrombolytics if they present within three hours. Carotid endarterectomy in patients with symptomatic carotid stenosis is more effective in preventing strokes in people aged over 75 than in younger patients.
