Ultrasonographic diagnosis does not improve outcomes in appendicitis

Although ultrasonography has been used in the diagnosis of appendicitis for more than 20 years, there have been no adequately controlled studies performed to assess the influence of ultrasonography on clinical outcomes. On page 919 Douglas et al report on a randomised trial of 302 adults and children with suspected appendicitis. They found no reduction in adverse outcomes (non-therapeutic operation or delayed treatment in association with perforation) or in duration of hospital stay. Safe management of appendicitis still requires good clinical diagnosis.
