Surgery reduces risk of fracture in primary hyperparathyroidism

Patients with primary hyperparathyroidism have an increased risk of fracture, but the effect of surgical treatment on this risk has not been evaluated. Vestergaard et al (p 598) studied 674 patients with primary hyperparathyroidism before and after parathyroid surgery and matched controls. The risk of fracture was increased among the cases up to 10 years before surgery, mainly due to more fractures of the vertebrae, distal lower legs, and proximal forearms. After surgery the overall risk of fracture was the same as in controls. However, a small increase in risk of distal forearm fractures appeared more than 10 years after surgery.
