Patients with stroke benefit from domiciliary occupational therapy

Patients with stroke who are discharged home from hospital often encounter unanticipated problems in adapting to the home environment. Home based rehabilitation has been proposed as one strategy to ease these problems. On p 603 Gilbertson et al report the results of a randomised trial of a brief programme of domiciliary occupational therapy, which resulted in initial improvements in daily activities, although the benefits were less apparent at 6 months' follow up. Extending rehabilitation into the postdischarge period can enhance both patient recovery, at least initially, and patient satisfaction.
