In their fortnightly review on antidepressants in depression (p 1188)
Spigset and Mårtenssen come to similar conclusions to Malt et al. The
evidence suggests that most patients with major depression are best
treated with a combination of antidepressants and psychotherapy and
that in general no drug is clearly more effective than another. Their
suggested approach is to prefer newer antidepressants in mild and
moderate depression (where side effects are more important to patients)
and tricyclic antidepressants in more severely depressed patients,
though the patient's response to drugs in a previous episode remains a
valuable guide.
