Cerebral organic changes, such as deep white matter lesions detected by
magnetic resonance imaging, have been hypothesised to be associated
with depression in late life but their effect on the outcome of
depression is unknown. O'Brien et al (p 982) followed 54 elderly
people with major depression for three years. The presence of severe
deep white matter lesions was significantly associated with a poor
prognosis.
