This week’s quiz is on dementia and is taken from the OnExamination revision questions for the MRCPsych exam paper 1 and 2.

What is the diagnosis in each of the following four clinical situations? Each option can be used once, more than once, or not at all.

A A man presents with sudden onset dementia accompanied by a frightening loss of vision.
B A 56 year old man presents with slowly progressive dementia. His wife complains that he is increasingly disinhibited and socially coarse. At times he is euphoric, but he is also apathetic and difficult to get "going" at other times.
C A 52 year old opera singer presents with rapidly progressing dementia accompanied by myoclonus, extrapyramidal signs, and cerebellar ataxia.
D A 65 year old woman presents with rapid onset fluctuating cognitive deficit and is distressed. Clinical examination reveals no focal neurological problems.
