EDITORNewland et al point out that patients who have had a splenectomy should know the risks, be immunised, and take prophylactic antibiotics.1 But what should happen for patients who were operated on a long time ago (more than 20 years) who have never had any vaccines and are not having antibiotic treatment? No guidelines exist for such patients.
