
Preface
This guideline was developed over a period of 18 months in 1994/5. The group members committed both time and enthusiasm to the task and I am immensely grateful to them for this. Guidelines evoke differing responses from different people; if I were to have any influence on how this guideline was viewed I would wish it to be viewed in the following way:
"Medical practitioners should regard this guideline as a useful reference tool: not the rulings of philosopher kings, but the attempt of thoughtful people to share their knowledge - albeit imperfect - with other people."
